Home From Hospital
Five Lamps are providing a FREE support service for up to 14 days for over 50s in the Stockton Borough! The Home from Hospital Project can help to support in many ways such as:
- Helping get back on your feet in your own home
- Shopping for food and essentials
- Ensuring the heating works
- Tidying the house, social and welfare visits
- Collecting and delivering prescriptions and medicines
- Awareness of local referral organisations and will refer on for further support
- Supporting customers getting to GP & hospital and other appointments following discharge
