Requesting access to your information
The Practice is registered and complies with the General Data Protection Regulations. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the law.
Requests must be made in writing which includes email requests.
We are now able to email copies of your records to you but will require completion of a form to give us permission to do so.

How we share your records
The NHS shares your data under strict controls to enable better health care. However, you reserve the right to opt out of this data sharing.
